Seminar Registration
Directions:
- The "Click Here to Pay" button at the bottom of this page will take you to PayPal for payment. Before you get there, though, please take a moment to fill out the form below. This class includes a free T shirt, so please indicate your size and style preference (pictures below, ladies is the V neck). Be sure to click the Submit button!
- If you have a Promotion code, please fill it in. Initially you will be charged the full amount until your registration is reviewed by a human, and then the discout will be applied and refunded.
- The form you just filled out will be replaced by a Thank You and a Back button. Ignore Back button.
- Click the Click Here to Pay button to pay via PayPal. Registration is not complete (and your place in class isn't assured) without this step. If you entered a discount code in the form, the appropriate amount will be refunded on your credit card when verified (usually within 2 business days).
- Mark the date in your calendar book. Also note the location: The Feminist Karate Union, 1426 S Jackson St, 3rd floor, Seattle WA 98144.
Now please download and fill out this Class Particpation Agreement (click here for the form), and bring it with you to class. Doing this in advance will greatly help your class begin on time.
If the class participant is under the age of 18, a parent or legal guardian needs to sign.
